Responsibilities

This Primary Care APP role delivers comprehensive, longitudinal care to a geriatric population within a PACE (Program of ALL Inclusive Care for the Elderly) model. the provider works closely with an interdisciplinary team (IDT) to assess, plan, and manage care, ensuring continuity across all care settings.

The APP is responsible for performing detailed evaluations, managing, chronic and acute conditions, coordinating care plans, and supporting safe transitions of care. This role emphasizes thoughtful clinical decision making, minimizing unnecessary testing and referrals while maintaining high quality, patient centered outcomes.

Qualifications

Certified: California Family Nurse Practitioner License

DEA Certificate

BLS or ACLS certification

Graduate of an accredited Family Nurse Practitioner Program

National certification in his/her primary care specialty

Qualified to practice by certification in the State of California

Experienced FNP (particularly those who have worked in Health Centers) in primary care or geriatric care are preferred.

The salary wage range that UHC reasonably expects to pay for the position upon hire starts at $137,000.00 a year plus bonus. Our competitive salaries are dependent on knowledge, skills and experience. The base pay does not include other forms of compensation and/or incentives.
